EXPRESSION OF INTEREST- for the recommissioning of the service to deliver the Healthy Child Programme
The Croydon Public Health Nursing programme is being recommissioned to deliver the Healthy Child Programme, which is the national prevention and early intervention public health framework, providing a statutory duty to deliver the following services to all families and has a financial envelope of ?6.5m: (i) Health Visiting, including the mandated universal health visiting reviews (ii) School Nursing, including the mandated National Child Measurement Programme (NCMP) The mandated elements of the service are not the full extent of the offer from this service.
The recommissioned model is a needs-led model that responds to the increasing number of vulnerable families with complex needs and includes a health promotion model, which will allow for the transfer of health knowledge through community nurses into the wider community and build on capacity within the Early Years and Education sector.
The model provides an enhanced service that adds value by investing in the development and sustainability of the Health Visiting and Early Years workforce.
Croydon replaced the FNP service with an enhanced Health Visiting High Intensive Intervention tier that is universal in reach - personalised in response for vulnerable, complex families covering all ages and levels of gestation.
The service is expected to operate as part of the integrated Croydon Family Hubs offer.
Both of the Health Visiting and School nursing teams will include a skill mix of SCPHN and non-SCPHN trained staff, to enable interventions to be specialist-led and tailored to the level of need.
