Tier 3 Weight Management for Hampshire Clinical Commissioning Groups

A consortium of six CCGs across Hampshire are seeking to contract for the delivery of a Tier 3 Weight Management Service.
The aim of the Service, due to commence on 1 April 2016, is to ensure that high quality, multidisciplinary care is delivered to its patients.
Outcomes for weight loss, psychological well-being, quality of life and reduction in co-morbidities will be compensated under a risk-reward scheme.
The service is offered for tender in two geographical lots: LOT 1: East Serving practices of NHS North East Hampshire and Farnham, NHS North Hampshire, NHS South Eastern Hampshire, NHS Fareham and Gosport LOT 2: West Serving the practices of NHS West Hampshire and NHS Southampton City The aims of the service • Innovate to deliver the expected weight management outcomes • Implement best practice contained in NICE Obesity pathways and the guidance of the Royal Colleges • Treat those patients not wishing/unsuitable for bariatric surgery who have complex obesity, where intervention is appropriate to stabilize their condition and optimize their health and wellbeing, prior to the patient being referred back into primary care for onward treatment • Provide robust assessments to understand and treat issues beyond weight to establish best treatment options and management plan • Psychological therapies are integrated into the whole pathway with routine psychological screening being undertaken to determine patients requiring any long-term additional support or treatment from a psychologist • Assess and treat pre-bariatric surgery (Tier 4) patients in preparation for a surgical intervention Support to encourage health style Behaviour Change e.g. smoking cessation for a long-term solution and longer term sustainable self-management advice Liaison with other stakeholders expected (Multi-agency working): • Guarantee Systems are in place to ensure shared care with each patient’s GP • Regular contact with tier 2 and tier 4 providers to clarify thresholds for treatment and to focus effort as early in the pathway as possible The contract duration will be for 4 years with one 12 month optional extension period available to Commissioners at the end of the initial contract period.
There will be an initial year without the risk reward scheme to enable providers to build teams to provide a quality service.
The annual financial envelope for this requirement is £ 719,200 over the four years initial contract period for LOT 1 East. and £ 878,400 over the four years initial contract period for LOT 2 West.
The maximum tariff is set at £1,170.
For the purposes of the advert the maximum value has been calculated using the maximum budget available for both Lots over the contract period including extension options.
