The brief

North Staffordshire and Stoke on Trent Clinical Commissioning Groups are looking to commission, as a pilot for 12 months, a different way of commissioning home care packages for adult patients eligible for Fast Track Continuing Health Care services across Northern Staffordshire.

For those patients with a rapidly deteriorating condition that may be entering the terminal phase, the Fast Track process was set out in the National Framework to enable immediate provision of NHS Continuing Health Care.

It is the CCGs intention to commission care that improves patient safety and quality of care for those with a life expectancy of less than three months.

Moving from the traditional commissioning on an individual case by case basis, CCGs wish to commission, by means of a contract, around 40,000 hours of home care per year specifically to support the speedy discharge of patients from both acute and community hospitals to enable patients to be cared for at home if this has been identified and documented as the preferred place of care and death.

Providers will be required to deliver person centred care in line with national guidance and local requirements in a patient’s own home, including but not exhaustive of assistance with: • Medication • Nutritional management • Getting up/going to bed • Dressing • Washing and bathing • Continence management and aids • Grooming • Listening and support with social and/or emotional needs Contracts will be awarded to a maximum of two providers across the registered population of North Staffordshire and Stoke-on-Trent inclusive of rural locations.
