Calderdale CCG: Pulmonary and Podiatry Clinic Transport Service
Calderdale CCG are wishing to hold a market test for the provision of transport for COPD and Podiatry patients attending for treatment at specified community venues as well as ad-hoc transport to secondary care provision.
Some out of area transport may be requested.
The requirement is for transport services to be planned, booked and provided in a way that ensures that people receive the most appropriate and effective service possible.
Standard booking arrangements that make the best use of information systems for the prioritisation and management of transport resources need to be provided.
It is intended that each patient should reach their place of treatment/appointment location and/or home in comfort and without detriment to their medical condition.
Patients will be collected from the home or other agreed collection point in a timely manner so that they will arrive on time for, or a maximum of 30 minutes before, their appointment or admission due time.
Patients should be collected from the healthcare facility within 60 minutes of the facility informing the Provider that the patient has completed their treatment.
Journey times for each patient should be no more than 60 minutes unless it is for transport out of area.
A variety of transport options will be required to meet a person's needs including car, minibus and passenger assistance as required.
Carriage of equipment is also expected alongside minimal additional luggage.
The service operates Monday to Friday within normal working hours and will accept people of all ages.
Those people aged 16 years or younger will be accompanied by a responsible adult.
