Bracknell Forest Council Public Health requires a Commercial Community based organisation to facilitate the delivery of a Tier 2 Weight Management Service across the Borough of Bracknell Forest.
The Service will be aimed at people who are in the obese category, in the adult population or those who are overweight with co-morbidity.
Organisations wishing to tender need to demonstrate that they have a strong understanding of what a tier 2 adult weight management service require.
The organisation must have the capacities, resources and expertise to deliver this service across the borough taking into account of the demographics and the health inequalities in the borough.
The organisation should also have the ability to demonstrate cost effectiveness over the life of the programme and beyond.
Aim of the service is to improve the health and wellbeing of overweight and obsess adults by supporting them to achieve and maintain a healthier weight and improve physical fitness through making appropriate and sustainable lifestyle changes to patients eating and physical activity habits.
The contract will commence 1st April 2018 and will for an initial period of 1 year with an option for a further year.
