NHS North West London Integrated Care Board ("NHS NW London ICB" or "ICB") required a Central London (Westminster) based provision of a Dementia Advisor (DA) to deliver post-diagnostic support, information and advice to people with a diagnosis of dementia.

The main focus is supporting people with a recent diagnosis, and/or in early stages, to live well with dementia, to help them to stay independent and access services in the community.

The DA also remains as point of contact for people with dementia from diagnosis through to end-of-life.

The post holder provides information on local support services for the person with dementia, providing advice and signposting as and when required.
