Calderdale CCG - Provision of a Tier 3 Weight Management Service
Calderdale CCG is seeking to procure a Tier 3 Weight Management Service.
The service will be delivered within the Calderdale footprint, according to the needs of patients and aligned to the principles of Care Closer to Home.
It is anticipated that this new service will accommodate a maximum of 90 people during the term of the contract.
By improving the management and monitoring of this patient group, the Tier 3 Weight Management Service aims to support participants in leading active lives in the community and avoiding unnecessary hospital based treatment or admission; therefore reducing dependency on hospital based services.
