Wellbeing Navigation Service - Market Engagement Event
Medway Council are recommissioning the Wellbeing Navigation Service for adults 18+, to commence 1 April 2023.
We are keen to engage with providers to help shape the new service.
Wellbeing Navigation (also known as Care Navigation) supports personalised care through a range of support for up to 12 weeks, in the home or community.
The Wellbeing Navigation Service is a coordination process and a key ingredient to achieve integrated care provision to improve health and well-being in Medway.
Wellbeing Navigators are usually based in a multidisciplinary team, to identify and signpost people to available services, acting as link workers.
The aim is to embed our Wellbeing Navigation service within the Health and Social Care system and support clients to manage their conditions so that they do not deteriorate or develop further clinical issues where possible.
Wellbeing Navigation empowers clients to 'help themselves' Medway Council will be hosting a provider engagement event on Microsoft Teams on the 23 August 2022 between 10 am and 3 pm.
We encourage all current and prospective providers to attend.
