The brief

NHS Leeds South and East CCG wish to contract with providers for a Social Prescribing Service for patients registered with a Leeds GP Practice.

Brief Description of Service Requirements: The aim of the service is to facilitate, manage, assess and signpost service users to community based services for patients requiring Social Prescribing.

Social Prescribing is often referred to as: ‘A means of enabling primary care services to refer patients with social, emotional or practical needs related to their health and well-being to a range of local, non-clinical services, often provided by the voluntary and community sector, and to broader universal services’ Age Concern (2011).

The objectives of the service are: — Increase the number referrals made by GP's, health, social care and other relevant professionals to local community activity provision, targeting communities most in need and leading to more local people accessing local community activity in areas of greatest need. — Increase the number of local people accessing local community activity provision. — Improve the social support available for individuals by supporting, signposting and connecting people to community groups, activity and provision in the local area. — Reduce social isolation and improve community connectiveness to contribute to overall improvements in the health and wellbeing of individuals and communities. — Apply to all residents of LSE, of all ages and operates a flexible, inclusive service to meet diverse needs.

All provisions of this service will be expected to meet and comply with NHS standards for IT Governance, Data Protection, security of general and personal information, data and records held and processed and transmitted (via and over all mediums).

The successful provider will be required to sign the NHS Standard Contract for the service and accept all terms and conditions.

Potential Providers are asked to please note the Following: — Service specifications, Standard NHS Contract and the electronic application form can be found online at www.nhssourcing.co.uk — Applications will be received until 3.7.2015 — Applicants are advised to review the service conditions and general conditions for the NHS standard contract before making their applications, these can be found at the following link: NHS England 2015/16 NHS standard contract. — If applicants are asked clarification questions during the evaluation stage they will have 48 hours to respond.

The contract will last for 3 years.

As the procurement falls under Part B of Schedule 3 of the Public Contracts Regulations 2006, it is only mandatory for the procurement to comply with Regulation 5 (2).
