Nene and Corby Clinical Commissioning Groups are seeking to procure a Community Diabetes Multidisciplinary Team Service in Northamptonshire.

The aim of the community diabetes MDT service is to deliver a best practice care model linking with both primary and secondary care that provides the best possible outcomes for patients.

This will be achieved through the early identification of diabetes, provision of education and specialist care for patients outside an acute setting, in order to avoid complications and reduce avoidable hospital admissions and attendances, whilst supporting patients to effectively self-manage their condition in the community.

The Commissioners would like to invite potential providers to a market place event to be held at Francis Crick House, Summerhouse Road, Moulton Park industrial Estate, Northampton, NN3 6BJ on Wednesday 17th February 2016 at 11.00am.

Places are limited to two representatives per organisation.
