Prior Information Notice - Health and Social Care - Cumbria CCG
NHS Cumbria Clinical Commissioning Group (CCG) is inviting Expressions of Interest for “Health and Social Care Co-Ordinators” for the population in Carlisle.
These roles will work with practice teams, to proactively manage patients in a community setting and therefore avoiding unscheduled admissions.
The funding for these posts is time limited until 31st March 2016.
The ‘Out of Hospital ‘strategy 2015/16 for North Cumbria delivers the outcomes required as part of the “Together for a Healthier Future” programme.
We are doing this by working together to provide a whole system approach across secondary and primary care, social care, community therapy, third sector and housing services.
In line with the strategy the NHS CCCG is seeking to arrange local community services around Primary Care Communities.
To ensure services can deliver the care in the right place, at the right time by combining as many community services as a patient may need in helping to support their care away from hospital and to live independently.
This whole system approach will see patients at the centre of care planning and delivery support.
This will result in a greater focus on ill health prevention, provide proactive care when it is needed and help maintain patients’ independence for as long as possible.
Out of Hospital brings together health and social services and looks at the needs of patients as a complete picture reducing the number of fragmented interventions, making patient journeys shorter, safer, more effective and entail fewer ‘handovers’ between services.
This will also mean the greater involvement of patients in their own treatment and care, and will support the improved personalised coordination of care.
Health and Social Care Coordinators will be a critical role in the out of hospital system.
They will navigate and coordinate services for the patient at risk of unscheduled admissions, by providing timely support at a primary care level and interface/coordinate with social care and community services as necessary.
The intention of this notice is to inform potential providers they can express an interest in this service.
Please also be aware that existing similar services that are in operation in Cumbria are not affected by NHS CCCG’s requirements.
