The brief

North Staffordshire and Stoke on Trent CCGs have undertaken the design of a new service model for an Integrated Urgent Care Centre inclusive of a GPOOH provision for the population of Northern Staffordshire; this is in line with best practice nationally and aims to reduce the number of admissions into acute services.

This is a service to: a) Stream all self-presenting patients arriving at the ED at the Royal Stoke to determine whether they have a need that can be met in a primary care environment; this in turn will aim to reduce demand on ED services; b) Provide access for patients with an identified primary care medical need who initially present at ED; c) Provide urgent access to a GP during the out of hour's period for patients assessed with a primary care need by the NHS111 service.
