The brief

SCC is commissioning a service to incorporate a range of community based services which promote peoples' independence, and offer preventative and enablement support in the communities where people live.

It will be required to be accessible by anyone over the age of 18 years who requires support to maintain their independence and wellbeing as a result of their mental health.

It will promote the principles of social inclusion, prevention, recovery/ enablement and independence.

The Service will provide seamless, person centred support in communities to improve the mental health and wellbeing of people across each geographical service delivery area of the county and will help to prevent entry into more acute health and social care settings.

A variety of options will be available to those requiring support so that needs can be individually catered for and services will be accessible to all those who require it regardless of where in the county they live.
