Improving transition assessments and support for young adult carers to make positive transitions
This project will aim to improve the variation in practice around respite care / carers breaks across England, through sharing best practice, increasing availability of choice of quality services to enable carers to take the breaks they need and supporting carers to use personal budgets for breaks/respite.
In doing so increase the transparency so carers know what they are entitled to, and ensure greater consistency in what is available.
A choice of appropriate and good quality care needs to be available or carers cannot get the breaks they need for their own health and well being.
