The service will provide an Early Supported Discharge Service as well as a Community Stroke Rehabilitation Service which will support patients who have had a stroke to return to their homes at the earliest possible opportunity in order to continue their rehabilitation.
The integrated community stroke service will have close working relationships with acute stroke services, community services and primary care.
