NHS Chorley and South Ribble CCG and NHS Greater Preston CCG Bed Based Intermediate Care Service

NHS Chorley and South Ribble Clinical Commissioning Group and NHS Greater Preston Clinical Commissioning Group are commissioning a number of nursing care beds for patients that require 24 hour supervision during their recuperation, this provision will be across a number of sites (3 separate sites are planned initially, with 5 beds in each site) enabling people to be as close to their community as possible, offering patient centred care that prevents admission, is clinically effective, outcome focussed and supports independence.

The aim is to facilitate the transition to functional independence where possible so that each patient can return to their usual place of residence within a pre-defined period of time as detailed in their care and support plan.
