The brief

‘TRANSFORMING LIVES FOR VULNERABLE PEOPLE’ NHS Pennine Lancashire Clinical Commissioning Groups (Blackburn with Darwen & East Lancashire) are embarking on an ambitious and innovative procurement programme to further develop liaison services for vulnerable people from the current service provision to a more cohesive and integrated model of service delivery.

The objectives of the new service will be to: • Help individuals to access local resources appropriate to their need • Help individuals to build resilience by supporting people to identify solutions that address unsafe behaviors through intensive Case management care plans. • Develop individual pre-crisis plans which avoids escalation to unnecessary attendance at ED and UCCs, whilst also providing an urgent response as necessary when people do attend. • Supports individuals to make choices that promotes them to gain and maintain employment or education • Be sensitive of the inter-relationship between physical and mental health ensuring parity of esteem.

Context: This is an exciting opportunity has arisen to provide a new innovative service to transform the lives of vulnerable people across Pennine Lancashire.

Health and Social Care Commissioning Organisations have joined forces to address the complex needs of vulnerable people that span both health and social care into one effective multi-disciplinary service.

The 5-year forward view highlights the need for all age, 7 day a week services cutting across both mental health and physical health care.

Integrated assessments, which result in co-produced, care and support with outcomes that address unsafe life choices for individuals and consequently reduces the demand on acute service provision.

Service Requirement: Commissioners through this advertisement seek to start the expression of interest phase of this particular procurement process.

Commissioners require a service that will provide an intensive case management model drawn together from a broad range of providers working collaboratively together either as a single unique organization form or as a group of multiple providers with the appropriate governance to deliver truly integrated services.

The service will provide a response to people who attend in crisis at Emergency Departments and Urgent Care centres across Pennine Lancashire, as well as focusing on a case management approach to break the cycle of repeat attendances.
