SoEPS on behalf of Brighton & Hove CCG would like to invite potential service providers to respond to the Request for Further Information (RFI) for the provision a Memory Assessment and Support Service.

A high level summary of the proposed functions of Memory Assessment and Post Diagnostic Support service is as follows: a) Assessment & Diagnosis: b) Medicines Optimisation: c) Post Diagnostic Support This support may contain a number of elements: I.

Supporting Primary Care: II.

To provide access to a single point of contact for people with dementia and their carers, operational five days a week.

III.

Responding to early detection of functional change: IV.

Post diagnostic support should be delivered within the context of multi-morbidity.

V.

To promote support and inclusion for people with dementia and their carers using the service through provision of support/access to purposeful, stimulating and meaningful activities.

Advanced Care Planning.

Additional Interventions. d) Dementia Friendly Community: Take a lead in ensuring the continued recognition of Brighton & Hove as a Dementia Friendly City by raising the profile of dementia and reducing stigma.

This is a process designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a Tender exercise.
