GP Services (APMS) for Sollershott Surgery, Letchworth Garden City, Hertfordshire
NHS Arden and Greater East Midlands CSU (AGCSU), on behalf of NHS England Midlands and East (Central Midlands) and East and North Herts CCG (the Commissioners) under joint commissioning arrangements, is inviting suitably qualified and experienced providers to deliver Primary Care Medical Services under an Alternative Primary Medical Services (APMS) contract at Sollershott Surgery, Letchworth Garden City, Hertfordshire, SG6 3JW.
The successful Bidder will need to demonstrate that they can deliver service(s) of a continuously high-quality and will be responsive to the needs of the local population.
The Provider will be assessed on an on-going basis to ensure they deliver the quality required.
The services must be GP led and provided by GPs, Nurses and other clinical professionals or healthcare workers.
The Provider will be expected to: - Provide primary care services which will be accessible, convenient and responsive - Deliver all services throughout core hours (Monday to Friday 8am-6:30pm) - Design services around the needs of patients and carers, ensuring they are offered more choice and have a greater say in their treatment - Have processes in place to evaluate and continually improve on patient satisfaction - Have processes in place to engage effectively with the local communities - Participate in Quality and Outcomes Framework (QOF) - Be eligible for and expected to provide all Enhanced Services, remunerated in line with the relevant Directions as specified by the commissioner on an annual basis. - Promotion of healthy living with the aim of tackling the underlying causes of ill health.
As a result of this procurement exercise an agreement will be established with the successful Bidder(s) for a period of 3 years, with the Commissioner having the option to extend the Contract for up to a further 2 years (maximum contract period 5 years).
The actual patient list size was 5208 in October 2017.
The Commissioners believe that there is significant opportunity for practice development and to increase the patient list size.
Transitional funding will be available in the first year.
The anticipated financial envelope for Sollershott is approximately £2,445,973 (excluding transitional funding in Year 1) over the lifetime of the contract (5 years), with the opportunity to earn extra income via QOF and CCG GP Enhanced Commissioning Framework.
This opportunity will be open to the entire market.
As this tender is for existing services, the services must be fully operational by the service commencement date of 1st July 2018 (current contract ends 30th June 2018) in order to ensure continuity of care for patients.
The Commissioners would particularly welcome interest from forward thinking prospective providers wanting to deliver primary care services in a different way, working with the Commissioners and using the various significant support streams available and opportunities for local partnership working.
It is envisaged that TUPE will apply.
