The brief

Using the flexibilities of the Better Care Fund, Frimley ICS and Bracknell Forest Council are looking to commission a contract to deliver a Home from Hospital service.

This contract will cover eligible patients within Bracknell Forest who are being discharged from hospital.

The service will be managed by a coordinator who will liaise with key stakeholders to receive and action referrals.

The project will also work closely with the Intermediate Care Teams with referrals coming through from Intermediate Care as well.

The Council is currently undertaking market engagement prior to a potential procurement process being done by partners under the Better Care Fund.
