If the Health Trainer Provider(s) feels that the referred individual does not have the motivation to change due their lifestyle or a particular inhibitor in their circumstances or that they present a risk to themselves or others this can be considered an exclusion and would need to be referred back to the key worker with the reasoning) 4.
Design and implement referral documents, health trainer documents, and promotional materials 5.
Develop a monitoring and evaluation processes to monitor progress of individuals
