Lot 1 - Complex Emotional Needs - Managing Distress The service will cover Nottingham City, Nottinghamshire County and Bassetlaw and will support people who present in secondary care with Complex Emotional Needs (CEN), who need a specialist, enhanced support offer in the community by: • Supporting and sustaining individual recovery. • Enable individuals to build personal resilience to understand the factors that adversely affect their mental health. • Help people to have as much control over their lives as possible. • To improve health literacy and reduce health inequality. • Promote health, wellbeing and self-care with a view to improving both mental and physical health care. • Ensure a good quality of life i.e. greater ability to manage their own lives, stronger social relationships, a greater sense of purpose, the skills they need for living and working, improved chances in education, better employment rates and a suitable and stable place to live. • Promote social inclusion and reduce social isolation.
Lot 2 - Person-centred stabilisation - Nottinghamshire County For Patients within the Mid-Nottinghamshire (Mansfield, Ashfield, Newark and Sherwood), Bassetlaw and County South area (Broxtowe, Gedling and Rushcliffe) who do not specifically meet the requirements of secondary care/IAPT but do require support. • a person-centred approach to mental health support that is client led and meets the need of the individual requiring stabilisation. • a holistic approach to address issues around modern living skills, sustainable relationship's, understanding self and others, personal responsibility, confidence and self-esteem and ability to manage healthy lifestyles. • meet differing cultural and religious needs, understand the impact of discrimination and feelings of 'otherness' on an individual's lifestyle choices; understand the impact of poverty on an individual's lifestyle choices and be a safe place to openly express difference • Provides relapse prevention with a focus on well-being and coping skills. • Provides pathways to positive engagement in the community including signposting to other services where needs are identified such as support to families, student population needs, employment support and local communities etc. • Focus on recovery • Referrals to be received from secondary mental health services such as LMHTs, Crisis Teams, Primary Care Mental Health Practitioners and Liaison Teams.
