The health improvement service will inform, enable and support local people to improve their health and wellbeing.
Through universal, targeted and specialist offers of evidence-based interventions the service will deliver person-centred and appropriate help in a variety of settings and through a variety of methods that are relevant to people throughout their life course, to achieve improved health and wellbeing and to reduce health inequalities across Plymouth.
In brief the integrated service will cover; 1) Helping people to stop smoking tobacco 2) Helping people to eat healthily and to achieve and maintain a healthy weight 3) Helping people to be more physically active 4) Helping people to drink alcohol responsibly and safely 5) Helping people to achieve and maintain good mental health and wellbeing 6) An outreach service to provide NHS Healthchecks to target population groups
