Fast Track Continuing Health Care services

The aim of this service is to provide quality care in their own home for patients who have entered the terminal phase of life, and for whom a clinical has determined they should receive care under the fast track provisions.

Fast track care will be provided so as to achieve the following overarching aims: • To support the CCGs to enhance the quality of life for those in receipt of fast track care. • To deliver a high quality service ensuring service user dignity and respecting individual physical and cultural needs. • To work in partnership in the delivery of the service with other health care professionals, including (but not exclusively): • Local authority • Local acute NHS Trusts • Local community services providers • General Practitioner • Registered providers of care. • To achieve the performance standards set out in this specification • Hospices • Marie Curie…
