Cauldwell Medical Centre, Bedford Hospital NHS Trust, Kempston Rd, Bedford MK42 9DJ is in Bedfordshire and is part of Bedfordshire Clinical Commissioning Group (CCG).

The CCG currently has a provider in place to deliver general practice medical services at the surgery until 31st March 2020.

Bedfordshire CCG now wishes to appoint a suitable organisation to provide general practice medical services (under an APMS Contract) at Cauldwell Medical Centre from 1st April 2020 for a period of 5 (3+2) years.

Bedfordshire CCG aim to help ensure better outcomes for people by creating the settings for person-centered, personalised and co-ordinated care to thrive locally.

Over the life of this contract the CCG are seeking a provider with a track record of working with CCGs and local partners to: - Provide flexibility so that new models of primary care delivery can emerge, potentially during and beyond the life of the contract - Support the local Primary Care Network with their plans for integrated care - Be part of the local solution to help sustain resilient essential services, as well as with those looking to change models of care delivery to improve patient care and efficiency.
