Dementia Diagnosis & Assessment Service
We are seeking expressions of interest from organisations that can provide assessment and diagnosis for adults awaiting a dementia diagnosis.
We are looking to commission around 250 assessments although this figure could fluctuate.

An outline service specification is below : Outline Service Specification Deliverables: a) Deliver dementia diagnosis assessments including written outcome reports for all clients b) Provide Trust staff with access to a secure online referral portal. c) Provide service user experience outcome measures for all who have taken part in the service. d) Provide an outcome report on completion Assessment for the patient and Trust staff.
The report will be made available via the provider's secure online referral portal.
Aims: Provide support to Humber Teaching NHS Foundation Trust's (Trust) waiting list reduction plans by providing greater access and choice for adults in Hull and East riding of Yorkshire waiting for a Dementia Diagnosis.
Clients would already have been triaged by the Trust service and then passed on to the successful provider.
