he Contracting Authority awarded a contract following a Request for Quote (RFQ) process for a Specialist Assessment for Frail and Elderly Service.
The RFQ process was undertaken due to the service being under the EU procurement threshold for services detailed within Schedule 3 of the Public Contracts Regulations 2015.
The Authority will award a one year contract to provide the services within the North Lincolnshire CCG locality with the option to extend up to 6 months .
The contract will be paid on an incentive based payment basis; 90% of the tender price is stated as £318,450, with up to an additional 10% uplift being paid on the achievement of KPIs documented within the service specification (released at the point of the RFQ).
The aim of the service is to drive improvements in the way in which the care of older people is managed and co-ordinated to reduce A&E attendances and unplanned admissions into secondary care and improve the quality of life for older people.
The service will deliver comprehensive reviews of medical, functional, psychological and social needs of patients referred into the service who are assessed as being at risk of frailty, based on the principles of comprehensive geriatric assessment as defined by the British Geriatric society (2014).
Aims and objectives of service - To proactively identify people aged 65+ with moderate or severe frailty or dementia and support their needs through Comprehensive Geriatric Assessment and care planning. - To reduce the reliance on secondary care and acute care systems for people aged 65+ with frailty and dementia. - To enable self- management of frailty wherever possible and increase people's levels of independence through working collaboratively with health, social care and voluntary sector organisations across North Lincolnshire to ensure continuity of care across the pathway. - To identify and address training/education gaps for all staff involved in supporting frail and elderly people. - Ensure that patients and carers receive the right support from the right places and increase the number of patients and carers who have a positive service experience.
