The brief

NHS Calderdale CCG seeks expressions of interest from suitably experienced and qualified service providers who would like to be considered for the provision of a medicines optimisation service within NHS Calderdale CCG member practices.

The service is intended to begin in November 2017 with the aim of providing prescribing support to member practices through an appropriately qualified pharmacy workforce to promote and support the delivery of medicines optimisation.

The objective of the service is cost-effective, safe and high quality prescribing for the people of Calderdale delivered within our available resources.

The desired outcomes for the service are: • NHS Calderdale CCG primary care prescribing is within allocated budget • Cost-effective prescribing in line with best performing comparable CCGs • Improvements in quality and safety in prescribing • Improved compliance with the Calderdale formulary and SWYAPC antimicrobial formulary • Improved safety in prescribing of high risk medicines • Increased Clinician understanding of CCG, local and national prescribing priorities and initiatives The successful provider will ensure the delivery of the KPIs within the service specification.

Effective communication at all levels will be crucial to service delivery and clinical governance arrangements will be robust to ensure patient safety is paramount.
