The brief

This is a Prior Information Notice (PIN) and Request for Information (RFI) published on behalf of Greater Huddersfield & North Kirklees Clinical Commissioning Groups. (The commissioners).

The purpose of this PIN/RFI is to gauge market interest and potential for introducing the development of a lead provider with the responsibility to ensure delivery of a comprehensive care service to meet the needs of those adults (over 18 years) at the End of Life.

The commissioners are seeking expressions of interest from suitably qualified and experienced organisations to further inform their plans to deliver a comprehensive care and support service for people (and their families) at the end of life (as determined clinically and recorded to the patients EPaCCs / care plan).

Care will be provided in the persons preferred place of care such as their own home, care home, hospice etc.

Patients will be registered with either a North Kirklees CCG GP or a Greater Huddersfield CCG GP and resident within Kirklees.

The statutory duties of the CCG delivering Fast Track end of life care will remain with the CCGs with service delivery via a service specification and Memorandum of Understanding.

End of life care has been identified as a priority programme for the commissioning partners of Kirklees.

The Kirklees end of life strategy aims to deliver ‘High quality, person/family centred care for people at the end of life’ The service will operate, providing seamless high quality services to the population of Kirklees, demonstrating interdependencies with the community service provider, individuals GPs, hospital services, social care, independent and voluntary sector provision.

Services will be delivered to meet the following care pathway objectives:  People are informed as early as possible about the approach of end of life to enable informed decision making about their preferences.  End of life care is timely, compassionate and reflects their needs and wishes as far as possible with respect to physical, social, psychological, cultural and spiritual aspects.  People during end of life phase remain in a place of their preference where possible avoiding unnecessary hospital admissions.  Pain and other symptoms are managed as effectively as possible.  All people in Kirklees die with dignity and where possible, in the place of their preference.  People and their carers feel supported both during end of life care and after the person has died  People and their carers are engaged in the co-production of services and service developments linked to end of life care.  People are clear who they need to contact should they need support and advice For further information and criteria please see attached document - GHCCGEoF - additional information and criteria
