North Staffordshire and Stoke on Trent Improving Access to Healthcare for the Homeless
North Staffordshire and Stoke on Trent CCGs are considering the procurement of a service to improve access to healthcare services for the homeless population within Northern Staffordshire and Stoke on Trent.
The service shall provide qualified nursing support, on an outreach basis within community locations suitable for the local homeless population.
The service will need to provide a range of interventions that span physical and mental health and will require highly skilled and experienced staff.
The service will need to work closely with local partner organisations to ensure a coordinated multi-agency response for this complex patient group.
This notice intends to identify suppliers who may be interested, expressing an interest for this service.
The CCGs are initially asking for organisations to register their Expressions of Interest only at this stage by email to Elizabeth.johnson1@nhs.net by Friday 11th May 2018 This will not be advertised in OJEU as is below the required threshold.
