The brief

The service will provide individualised sub-acute healthcare to patients in their own home who are not medically fit for discharge, but who are clinically stable and can have their care delivered in a less acute setting.

Care will be delivered by a range of registered and unregistered healthcare professionals, working closely with hospital physicians, surgeons, therapists, nursing staff and other community health and social care practitioners to deliver interventions typically restricted to acute hospital settings.

The service pathways ensure a seamless transfer of care from the hospital to the home setting.

Patients remain under the care of their hospital Consultant whilst receiving the CARE at Home Service (CAHS).

A care plan and estimated discharge date are agreed with the KGH Consultant team prior to the patient transferring home.

The provider will liaise with the referring team should the care episode need to be extended beyond the estimated discharge date.
