COV - Short Term Residential Reablement Services (Cancelled)

This tender is for a Short Term Reablement support service, provided in registered residential care homes.

The service has been established to discharge individuals from hospital, for a temporary period of up to 6 weeks, with the aim to re-able the individual to be as independent as they were prior to admission to hospital and return to their home.

The service will be for residents of Coventry, predominantly people aged over 65 years, who have completed an acute episode of care but are unable, at that point, to return to their usual place of residence.

Beds will be available under the contract to receive referrals from Coventry City Council, 365 days a year.

Registered Care Homes will be based within Coventry boundaries and be Care Quality Commission (CQC) registered (nursing and residential for older people).

There will be a requirement for frequent assessments to be undertaken at local hospitals (as detailed within the specification).
