NHS SY ICB - Doncaster Place - Dementia - PIN

The intention is to have a service model which supports people both prior to a potential diagnosis of dementia (including Young Onset Dementia), through the assessment process and following a diagnosis where they are able to step up/step down through multifaceted pathways depending on complexity and need at any time, underpinned through an alliance agreement to ensure providers work collaboratively.
