NHS Greater Huddersfield & North Kirklees CCG: Care Home Support Service Market Testing
This notice is to gauge market interest and potential for introducing a Care Home Support Service for the registered GP population of Greater Huddersfield and North Kirklees CCG (CCGs) residing in residential and nursing care homes for older people in Kirklees.
The Commissioners are seeking expressions of interest from suitably qualified and experienced organisations to further inform their plans which encompass a Care Home Support Service.
The CCGs would like potential providers to submit details of their proposed model.

The Commissioners are keen to create a collaborative approach between providers which supports true integration of services in line with the GP Five Year Forward View and New models of Care.
The provision of a local integrated Care Home Support Service is a key element of the CCG's plan to deliver care at /closer to home to the improve quality of care and reduce the need for hospital attendances and admissions.
Current service provision is delivered by a number of GP practices supporting each care home and a Care Home Support Service provided by Locala across the Greater Huddersfield area.
Routine community nursing care into care homes is provided as part of our core community contract.
The majority of patients are currently registered with either a Greater Huddersfield or North Kirklees CCG GP, however a very small minority of patients in Kirklees care homes may be registered with a GP practice in neighbouring CCGs.
The provider of the new Care Home Support Service will be required to have the ability to register patients for general medical services.
This Care Home Support Service will be pivotal in helping to drive change, working collaboratively with local healthcare providers and stakeholders to plan for, and co-ordinate on-going health care with the primary aim of keeping individuals well and/or stable.
This approach will be underpinned with robust care planning to ensure all care is optimised, delivered in the care home, and wherever possible, avoiding a hospital attendance or admission.

This is a high need, protected client group whose average life expectancy after taking up permanent residence in a care home is 18-24 months.
Therefore, demand on health services is continual.
There is high prevalence of multiple co-morbidities, such as heart disease, respiratory disease, dementia, as well common ailments associated with the elderly.
The Care Home Service will be required to develop key relationships with Greater Huddersfield and North Kirklees CCGs GP practices; Mid Yorkshire Hospital NHS Trust; Calderdale & Huddersfield Hospital NHS Trust; South West Yorkshire Partnership Foundation NHS Trust; Locala; Local Care Direct; Yorkshire Ambulance Service; Kirkwood Hospice; Kirklees Local Authority; GP Federations (Curo and My Health Huddersfield) and the voluntary sector.
