Midlands Partnership NHS Foundation Trust (MPFT) are looking to procure a provider who is able to provide a Serious Mental Illness (SMI) Physical Health Outreach Service Supporting and working in partnership with the Mental Health Teams within the Trust in the Staffordshire Care Group.
The Provision of the service is required to support and achieve a number of key objectives, supporting service users within the following geographical areas: - Burntwood, Lichfield and Tamworth - East Staffordshire (Burton and Tamworth) - Cannock (Cannock, Rugeley and Haywoods) - Staffordshire (Stafford and Seisdon) The Service is designed to: • Support people on the SMI register by providing annual health checks • Avoid unnecessary hospital admissions • Prevent, reduce or delay the need for long-term care • Reduce health inequalities and exclusion by improving accessibility to checks for SMI population The SMI Physical Health Outreach Service will supplement the current service by contacting and engaging with people on the SMI register who have missed appointments for face-to-face checks, with the aim to encourage them to attend their appointments.
Home visits can be carried out also where appropriate.
An assessment will be carried out by a clinical staff member before the SMI Physical Health Outreach Service attends the property, therefore the risk will have been assessed before the service attends their home.
