Pre-procurement Market Engagemnt Event - Lewisham Community Rehabiliation Beds
Lewisham Clinical Commissioning Group will be tendering for a contract to deliver community bed-based rehabilitation (intermediate care beds) for its' patients.
Bed-based intermediate care addresses those instances when a person's level of need means they require intermediate care in a setting other than their own home.
In Lewisham bed-based intermediate care is a time limited episode of care (of up to 6 weeks), commissioned by the CCG and supported by the Local Authority.
Patients are referred to bed-based intermediate care either as a 'step up' from home for assessment and rehabilitation - as an alternative to hospital admission - or as a 'step down' from the acute hospital for a period of assessment and/or rehabilitation before they return home.
Lewisham Clinical Commissioning Group (LCCG) currently commissions 25 rehabilitation beds (including 6 with double handed support) for patients resident in Lewisham and registered with a Lewisham GP.
The maximum length of stay in these rehab beds is six weeks, during which time the service aims to improve the patient's functionality and physical ability.
Rehabilitation care is led by a multi-disciplinary team from Lewisham & Greenwich NHS Trust and supported by the bed provider, so that care plans are also followed out of hours.
Anticipated service outcomes include a measurable improvement in patient functionality and reducing the risk of an avoidable hospital admission We anticipate that the initial contract period will be for a term of 3 years, with the possibility of extending the term up to a further 2 years beyond the initial contracted duration.
This will be subject to satisfactory performance and by mutual agreement between the Commissioners and the Provider.
The CCG wishes to make every effort to inform and engage with the market prior to launching this procurement.
