The brief

It is not feasible to discharge COVID positive patients back to their normal place of residence including care homes for risk of the infection spreading.

Our strategy of not discharging back to homes has seen a low number of deaths and spread of infection at the height of the pandemic.

This is approach is consistent with the Calderdale CCG submission in the NHSE Evaluation of Care Home Support Framework.

A separate, additional bed capacity is therefore required to support this cohort enabling: • rapid discharge of COVID positive patients who are medically fit for discharge from hospital and reducing the number of delayed transfers of care (supporting the local winter plans) • bed capacity within the Calderdale system for COVID positive patients to mitigate o any second wave and/or; o a peak as we enter the winter period.

In Calderdale we will continue to work closely with partners, CCG, hospitals, the Local Authority and the voluntary sector a partnership approach to support these requirements.

The Revised Hospital Discharge Service takes a multi-disciplinary approach to provide a discharge to assess model to move patients out of hospital into an appropriate community setting in order to complete further assessment and/or provide a package of care that meet their health and social care needs.

The service will provide additional transitional bed capacity for patients stepping down from hospital who are medically stable but require a further period of rehabilitation and support and are COVID positive.

A review of the profile of the patients requiring this service has resulted in enhanced residential care model The provision will comprise 8 beds with an ability to increase those beds to 12 if and when demand increases.

The provision will work flexibly to meet the needs and demands of patients being discharged from hospital.

The service will be provided by care home staff and supported by in reach from community nursing, therapies and a dedicated GP service.

The service will comprise 8 beds with an ability to increase to 12 beds as demand dictates.

The Additional Community Bed Base will operate a 7 day service 24 hours per day from September 2020 until April 2021, covering the Easter period.

Normal daytime hours will be 9am - 5.30pm.

Admissions and discharges will take place 7 days per week during daytime hours.

Admission and discharges outside these hours could be agreed by exception.
