Following a successful training programme delivered in 2021/22, which trained over 1000 people throughout the region, this training offer will aim to upskill voluntary sector staff, volunteers, health and social care professionals, care coordinators, health coaches and social prescribing link workers across West Yorkshire in delivering personalised care approached to address inequalities, meet the needs of our most vulnerable communities, connect people to what matters to them and change the relationship between people and professionals.

The aim of this benchmarking project is to present an opportunity to invest and upskill our workforce to deliver standardised personalised care approaches to reduce variation in the delivery of care with a focus on people living with long term conditions including mental health.

We are inviting bids from academic institutions but are open to considering other organisations with experience of research, to deliver an evaluation, benchmarking our current position, completing data analysis, engaging with our workforce, communications to support the project, and developing a robust evaluation mechanism to understand the impact that personalised care training and development is having on our workforce and communities.
