The existing service has identified significant opportunities within the East Riding’s adult population to prevent, delay or reduce the pressure on ASC through early intervention and timely personalised support to address and improve resident’s Mental, Physical and Social health, reduce inequalities and inequities resulting from Access and infra-structure issues in the area.
The Council is seeking a collaborative, innovative and altruistic partner organisation, experienced in working with people, communities and partners to deliver time limited, personalised support to residents in a variety of ways and locations.
The new service is smaller, both in value and staffing requirements, than the previous social prescribing contract and the previous social prescribing connector service.
