Contract for the provision of Accommodation and Support to citizens who are homeless at the point of discharge.

This will include but not limited to:• Provide good quality accommodation that meets the needs of the citizens including those who are bariatric, and wheelchair users.• Provide flexible person-centred support ensuring that support is delivered in response to the client’s needs and allows them to maintain accommodation and improve their health and wellbeing outcomes.• Provide the lead and support which will assist clients to navigate and gain access to appropriate services, including but not limited to health care, substance misuse, education and/or support to gain employment.The Contract will last for 5 years (unless terminated under the terms and conditions of contract) with the provision to extend for up to an additional 2 years subject to requirements, funding and satisfactory performance.
