The NHS Health Check is a national prevention programme which aims to reduce the chance of a heart attack, stroke or developing certain forms of dementia in people aged 40-74.
It achieves this by assessing the top seven risk factors driving the burden of non-communicable disease in England.
It also provides individuals with behavioural support and where appropriate pharmacological treatment to address these risks.
Commissioning of NHS Health Checks is a statutory requirement of Councils resulting from The Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch Representatives) Regulations 2013.
GP Practices throughout the borough of Cheshire East will be awarded contracts for Delivery of NHS Health Checks (including identification of patient eligibility).
