As part of the NHS Long Term Plan Ambitions, Living Well Derbyshire are working with Community Mental Health Teams, Local Authorities and the Voluntary Sector to create 'Provider Collaboratives' in local areas to support the improvement of outcomes and reduce out of area placements.

Once established, the Living Well Local Leadership Coordinator will support the voluntary and community sector to become a key element of Mental Health Support in the community across the Derbyshire footprint.
