The Council requires a Managed Service for the Completion of Social Care Reviews (the Review); these will predominantly be reviews 6-8 week post care package commencement, although it is envisaged that the Provider may be required to undertake other reviews, at different timeframes.
Reviews will be undertaken to a standard which is Care Act 2014 compliant.
This may be the first opportunity to review a newly implemented package of support.
Reviews must be undertaken to align with the Council’s ambition to support people to maximise their independence in their own homes, through an approach which builds on their strengths, and which identifies and utilises community support which is available in their locality.
This procurement is conducted under the OPEN PROCEDURE.
The Review allows opportunity for a review of outcomes, both in terms of the service offer to sustain and support people to live as independently as possible, and to make the most effective use of what can be provided locally to support people’s independence, where that is appropriate.
