The brief

Carers Hub Contract The London Borough of Hammersmith and Fulham are seeking suitable providers to deliver services to unpaid carers in the borough.

The Carers Hub contract supports us to meet our Care Act (2014) duties to carers.

Services that the Carers Hub currently provides under this contract are: • Carers assessments, reviews, support plans andpersonal budgets • Social events and peer networking/support • Information, advice and training on a range of topics. • Signposting to other services where appropriate • Forging partnerships with local charities and businesses to enhance the offer to carers.

Referrals for an assessment can be made to our Carers Hub from the Council, other professionals or as a self-referral.

Council social workers are also able to conduct carers assessments as part of their work with the cared-for, but our Carers Hub provider does around 80% of the total annual carers assessments.

We are holding a market engagement event 1pm-2pm Thursday 1st May online using Microsoft Teams.

We hope you will be able to join us but we will send the slides and a survey to any interested providers who are not able to attend.
