The North East London (NEL) Integrated Care Board (ICB) seeks a Strategic Delivery Partner to undertake a 6-month diagnostic phase that will shape the Care Closer To Home (CCTH) improvement programme.

The CCTH initiative promotes a "home first" model, aiming to: - improve outcomes and independence for urgent and emergency care users; - enhance efficiency and productivity; - generate tangible and cash-releasing financial benefits for the NHS and local government across NEL; and - address significant health inequalities and deprivation across North East London's diverse population and promote health equity by ensuring services are inclusive, culturally competent, and accessible to all communities.
