The services will be commissioned across three themed lots, with a strong emphasis on collaboration and integration.
All appointed providers must work in partnership to ensure a cohesive, coordinated, and seamless experience for both residents and referring professionals.
This includes maintaining robust communication across all lots.
The three lots are: •Lot 1: Supported Hospital Discharge •Lot 2: Community-led Mental Health Outreach •Lot 3: Community-led Wellbeing and Independence Given the breadth of expertise required, we welcome proposals from partnerships, consortia, or lead provider/subcontractor arrangements.
The Council values the unique identity and recognition of individual organisations and encourages solutions that preserve choice for service users.
Contract Value and Term The total maximum annual budget is £750,000, allocated as follows: •Lot 1: £200,000 •Lot 2: £150,000 •Lot 3: £400,000 Providers may bid for all three lots but will be awarded a maximum of two.
Providers should indicate, in the event of scoring highest in all three lots, which two lots they would have a preference to deliver.
The contract(s) will run for an initial term of five years, with the option to extend for a further two years, subject to performance and budget availability.
Providers will be expected to manage fluctuations in demand within the agreed contract value.
