Disgwylir y bydd cefnogaeth yn cael ei darparu mewn modd sy’n hyrwyddo annibyniaeth ac yn gwneud y gorau o ganlyniadau a bydd yn cael ei darparu yn unol â deddfwriaethau perthnasol, megis Deddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014 a Deddf Llesiant Cenedlaethau’r Dyfodol (Cymru) 2015.

Bydd y gwasanaeth yn darparu cyfleoedd gwasanaeth dydd a gwaith i oedolion ag anableddau dysgu a/neu anghenion cymorth awtistiaeth, yn cynnwys unigolion ag anghenion cymorth cymhleth.

Bydd angen y ddarpariaeth gwasanaeth i hwyluso ystod eang o gyfleoedd gwasanaeth dydd a gwaith i fodloni’r anghenion cymorth a nodwyd a buddiannau mynychwyr y gwasanaeth arfaethedig.

Bydd y gwasanaeth yn gweithio ar y cyd gyda budd-ddeiliaid i sicrhau bod y gwasanaethau a ddarperir yn adlewyrchu anghenion a buddiannau mynychwyr y gwasanaeth.

Bydd y darparwr yn darparu cefnogaeth o ansawdd uchel gyda phersonél profiadol sydd â sgiliau priodol i ddarparu gwasanaeth sy’n cynrychioli gwerth am arian ac sy’n darparu canlyniadau a nodwyd i fodloni anghenion y boblogaeth leol.

Yn seiliedig ar lefelau darparu gwasanaeth presennol, rhagwelir y bydd angen i unrhyw wasanaeth arfaethedig bod â’r gallu i ddarparu cymorth i oddeutu 100 o unigolion.

Er gwybodaeth, mae’r gwasanaeth presennol yn darparu dros 200 o ‘gysylltiadau’ bob wythnos ac mae ganddo’r gallu i ddarparu ar gyfer rhwng 50-60 o unigolion bob dydd yn y gwasanaeth.

Mae’r gwasanaeth presennol hefyd yn darparu cymorth gyda’r nos a dros y penwythnos a bydd angen i unrhyw fodel gwasanaeth yn y dyfodol fod â’r gallu i atgynhyrchu’r elfen hon o’r cynnig gwasanaeth.

Disgwylir y bydd unrhyw ddarpar ddarparwr gwasanaeth hefyd yn berchen ar neu â mynediad at yr isadeiledd angenrheidiol yn nhermau staffio a’r eiddo corfforol sydd ei angen i ddarparu’r cymorth gofynnol.

Fel y nodwyd uchod, mae gan nifer sylweddol o fynychwyr y gwasanaeth presennol anghenion cymorth cymhleth a byddai’n hanfodol sicrhau parhad mewn cefnogaeth a sicrhau bod eu trefniadau cymorth presennol yn cael eu hwyluso gyda’r amhariad lleiaf fel sy’n ymarferol wrth drosglwyddo darparwr gwasanaeth.

Disgwylir y bydd yr elfen hon o ddarpariaeth gwasanaeth wedi’i lleoli yn ardal Gogledd-orllewin Sir y Fflint.

Bydd hyn yn ategu gwasanaethau cefnogi cyfleoedd dydd a gwaith anableddau dysgu presennol eraill i oedolion o fewn Sir y Fflint ac yn sicrhau dosbarthiad cytbwys o wasanaethau cyfleoedd dydd a gwaith o fewn ardal yr awdurdod lleol.

Er y disgwylir y bydd lleoliad(au) corfforol y gwasanaeth wedi’i lleoli yn ardal Gogledd-orllewin Sir y Fflint, bydd cyfeiriadau ar agor i holl breswylwyr Sir y Fflint sy’n bodloni’r meini prawf cymhwyso ar gyfer y gwasanaeth.

If you feel you have the knowledge and expertise to deliver this service, please contact dawn.holt@flintshire.gov.uk Day and work service opportunities – learning disabilities and/or autism (adults) Flintshire County Council seeks to procure the services of a provider for the purposes of providing learning disability day and work opportunities support services for adults in the North West locality of Flintshire.

It will be expected that support will be delivered in a manner that promotes independence, maximises outcomes and is delivered in accordance with relevant legislation, such as the Social Services and Well-being (Wales) Act 2014 and the Well-being of Future Generations (Wales) Act 2015.

The service will provide day and work service opportunities for adults with learning disabilities and/or autism support needs, including individuals with complex support needs.

The service provision will be required to facilitate a wide range of day and work service opportunities to meet the identified support needs and interests of attendees to the prospective service.

The service will work collaboratively with stakeholders to ensure that the services provided reflects the needs and interests of the attendees of the services.

The provider will deliver high quality support with appropriately skilled and experienced personnel to provide a service that represents value for money and delivers identified outcomes to meet the needs of the local population.

Based on current service delivery levels it is anticipated that any prospective service would need to have the capacity to provide support for approximately 100 individuals.

For reference, the current service provides in excess of 200 ‘contacts’ per week and has the capacity to accommodate between 50-60 individuals per day at the service.

The current service also provides evening and weekend support and any future service model will require the capacity to replicate this element of the service offering.

It is expected that any prospective service provider will also either own or have access to the necessary infrastructure both in terms of staffing and the physical premises required to deliver the requisite support.

As noted above, a considerable number of attendees to the current service have complex support needs and it would be imperative to ensure continuity of support and that their present support arrangements are facilitated with as minimal disruption as is feasible in the event of a transfer of service provider.

There is an expectation that this element of service provision will be based in the NW locality area of Flintshire.

This will compliment other existing learning disability day and work opportunities support services for adults within Flintshire and ensure a balanced distribution of day and work opportunities services within the local authority area.

Although it is expected that the physical location/s for the services will be based in the NW locality of Flintshire, referrals will be open to all residents of Flintshire that meet the eligibility criteria for the service.
