Homeless Health Programme (Kensington & Chelsea and Westminster)
1) This model delivers a clinical outreach programme which takes healthcare services to individuals with a view to improving access to primary care for those on the streets.
With access to 4 commissioned hostel beds, the GP practices deliver a clinical programme which is responsive to changing need and circumstances.
The specialist GP practices work in partnership with one another and system partners (who provide assistance such as taking patients to their health appointments), hospital Health Inclusion Teams, CLCH, CNWL, Local Authorities, and Public Health.<br/><br/>2) Due to the nature of the health care services the existing providers are the only realistic capable provider for the relevant authority’s specific requirements, therefore direct award process A applies.
The relevant authority holds the view that these existing providers are uniquely positioned to meet the requirements of the service, and there are no viable alternatives capable of delivering the same level of quality and continuity.<br/><br/>Maintaining continuity of care is of paramount importance, as any change in provider and/or group of providers could lead to significant disruption of essential services.
Trust is critical to ensuring effective engagement and support, and replacing these providers could undermine that trust, ultimately compromising the quality of care provided.<br/><br/>3) The existing group of providers are;<br/><br/>Great Chapel Street Medical Centre and The Doctor Hickey Surgery – Specialist Homeless Health GP surgeries.<br/><br/>The Passage and Edward Alsop Court – Specialist Homeless Health hostel beds.<br/><br/>Groundswell – Specialist Homelessness support including peer advocacy.<br/><br/>2) An award has been made following Direct Award Process A<br/><br/>3) The length of the opportunity is 3 years and the dates between the services are to be provided is from 01 April 2026 - 31 March 2029.
