The brief

UK6 - Contract Award Notice.

The purpose of the Service is to deliver a countywide, community embedded Social Prescribing and Community Wellness model for residents aged 25 and over living in Wrexham County Borough.

In line with the National Framework for Social Prescribing, the Service provides a targeted, personalised, and non-clinical wellbeing intervention for individuals with identified holistic wellbeing needs.

The Service will offer proactive, therapeutic style, non-clinical interventions that go beyond simple information sharing.

Practitioners will work collaboratively with individuals to understand what matters to them, co-develop personalised plans, and provide hands on support to help people overcome barriers, strengthen resilience, and achieve meaningful wellbeing outcomes.
