The brief

The Enhanced Care Pathway (ECP) is an integrated proactive approach to delivering care for frail older adults.

It brings together a range of health and social care professionals to provide joined up effective care planning and delivery and to act rapidly when care needs escalate.

Based on national prevalence it is expected that around 3000 frail elderly patients registered with Wandsworth GPs will be identified to be managed within the pathway.

The procurement is a bespoke process adhering to the principles required for health and social care services falling under the Part B category of the Public Contracts Regulations 2006.

The Contracting Authority is not bound by the Public Contracts Regulations 2015.
